Phys Ed Medical Excuse
If you are out on a medical excuse you need to complete an assignment for each week you are out of physical education class. For example, if your medical note from your doctor states that you are out of physical education from 10/1-10/30. You need to complete 4 assignments because you are missing 4 weeks of physical education class. If you have any questions, please see your Phys Ed Teacher.
Medical Assignments
All medical assignments are published on Schoology.
You are responsible for completing in your medical assignments on the specified due date.
Any assignment that is submitted late will lose credit and assignments that are not submitted will receive a zero.
FAILURE TO COMPLETE MEDICAL ASSIGNMENTS WILL RESULT IN A FAILING PHYSICAL EDUCATION GRADE
Medical Note Procedure

Students will hand in their doctor’s note to the Nurse’s Office. The student’s Physical Education Teacher will receive a modified copy of this note. Students will continue attending their Physical Education class for attendance. Once attendance is taken, the student will then report to his/her medical study hall for the period. If students are NOT completing their medical assignments their Physical Education Teacher will keep them for class and require them to complete the work.
Students will need a clearance note to come back into Physical Education class.
